Position: Youth Advocate
Status: Part Time (24 hours per week)
Job Description: Responsible to attend Permanency Round Tables (PRTs) and family meetings to help youth navigate through the Department of Human Services system and to achieve permanency. The ideal candidate will have spent time in foster care as a youth in order to be able to relate to the youth on his/her caseload.
Supervised by: Youth Program Director
Qualifications:
- Time spent in foster care as a youth is required
- Demonstrates skills critical to engagement, advocacy and system navigation.
- Must be familiar with family systems and conflict management.
- Must have proven organization, time management and team partnership skills.
- Ability to respond safely to emergency situations.
- Must be able to transport and have a clean driving record.
- Must pass a FBI fingerprint screening.
- Skilled in online documentation and use of database systems.
Responsibilities:
The minimum performance expectations include, but are not limited to, the following functions/tasks:
- Engage youth and ensure ongoing contact through relational skills.
- Meet with youth prior to PRTs/family meetings if possible and provide follow up support as needed.
- Manage a caseload of assigned clients who need ongoing assistance and follow up
- Assist youth in identifying extended family and supports.
- Problem solve with youth in order to overcome barriers to success and experience progress on their treatment plans.
- Maintain routine contact with each youth.
- Take all necessary and reasonable precautions to protect confidential information.
- Model non-discriminatory practices in all activities.
- Comply with and support suggestions and policies set by Matthews House leadership.
- Complete all required intake and ongoing documentation in a timely and accurate manner.
- Actively participate in weekly supervision.
- Actively participate in weekly staff meetings, when possible with schedule.
- Maintain, in a confidential manner, complete and accurate client files.
- Perform other duties as assigned.
Specific Responsibilities:
- Maintain a caseload of 12-15 (part-time Advocate) or 20-25 youth (full-time Advocate).
- Contact youth within 24-48 hours of receiving a referral.
- Attend each youth’s PRT/Family Meeting.
- Initially maintain a minimum of one face to face contact with each youth per week and telephone contact as needed. This level of contact can be reduced as the case progresses and/or based upon the needs of the youth.
- Prepare the youth for the meetings and ensure their voice is heard in the meetings.
- Assist the youth and the treatment team in determining the permanency options for the youth.
- Assist youth in meeting their treatment plan goals and problem solve any barriers that may exist, particularly around permanency for the youth.
- Assist youth in identifying natural supports, including but not limited to, extended family, teachers, coaches, friends, family friends, etc.
- Invite youth to participate in IL and EE Groups at The Matthews House.
- Assist youth in learning about the community and the resources available.
- Maintain regular contact with the youth’s caseworker.
- Accurately complete billing requirements, including documentation of ROCs in SPOC, by the specified date.
- Accurately record appointments on the Google calendar.
- Check in with the Director or Team Lead at least once per week.
Special requirements:
Candidate must possess the leadership qualities and personal characteristics necessary for working effectively with fragile families. Must have solid relationship skills necessary to engage families and collaborate with professional service providers, including Larimer County Department of Human Services. Frequent operation of office equipment is required. Candidate must possess good moral character.
